I was gonna say ZOMG! As another possibility, at least graphically, though the battle system runs somewhat differently.
Besides, you can do it YOUR way if you want.

There's always Graal and a new one -- The Mana World Online. They modeled Mana World after Seiken Dentesu, supposedly. It's supposedly open-source as well. I think to get the source, though, you need to have GIT and go from there.

To answer to the opening poster (and i'm quite confused why no one has mentionned it yet), Ragnarok Online summarizes what you described. 2.5D graphics with 8-way movement, huge open world with player interaction and pvp etc. Its probably one of the oldest MMOs out there. Not that interesting either but the concept exists.

if you want more sandboxy shit, there's also haven & hearth, also wakfu which is in beta now and is actually kinda cool, it's like a combination of ff tactics and harvest moon.
