Don't you mean Malignant? If he's only just at the Titan Squirrel it's possible he hasn't done the quest to get Tumor (Malignant) yet, as it's pretty far away from the main story areas of Act III.

edit: oh wow im the second highest in the guild.. this reminds me of wow lol
